The technique of permanent thread lift, using tensor notched threads, is always non-surgical and reversible. If absorbable threads bring a very decent result for the young patients, undergoing a small and localized sagging, permanent threads have much better efficiency and durability. They offer a rejuvenation of appearance from 5 to 10 years.

Why lifting up the tissue ?

Because contrary to popular belief, the face undergoes only a very moderate melting of some adipose (fat) tissue under the skin. There is even an increase in total fat, superficial and deep, under the skin at least until the age of 80 years. Thus, the breaking of the oval, the collapse of the cheekbones, the appearance of jowls, nasolabial folds and marionette lines are in no way due to a phenomenon of disappearance of this fat called “hypodermis”. Rather, they are the result of a slip of that fat from its original location in the middle third of the face to the lower third of it. Take the test with a mirror and lie on your back, you will see that all these signs of aging have disappeared. This is a problem of fat distribution due to gravity and not a problem of loss of volume.

We must therefore use a technique to effectively and long-lastingly reposition the famous sagging tissues upward. Only thread lift by tensor threads (also called suspension or notched threads) is capable of without surgery.

Dr. Marc Benatar, clinical director at Nice, has appropriately presented in 2013 a final thesis on this subject with the study of 100 patients examined by MRI.

This medical facelift is REVERSIBLE :

Indeed, although there is absolutely no reason for this to be considered, it is still possible to GO BACK choosing to do REMOVE THE THREADS, even years after the procedure. The thread will then be removed by your doctor and your face will simply regain its original appearance. With the injection of an anesthetic that will moisturize the subcutaneous tissues, the removal is done with ease. The thread, as during its installation, does not damage any tissue and creates no mess being extracted. There is also no major fibrosis around the thread.

The set up as the eventual withdrawal of thread leaves no trace on the patient’s face. The choice of non-surgical facelift by permanent thread is by no means a commitment for the rest of your life.

The term “permanent” can be scary and I understand that. It simply reflects the perfect biocompatibility of the product and means that it remains perfectly intact in the patient’s face.

A 5 to 10 years rejuvenation :

Thanks to the permanent suspension thread named Infinite-Thread®, which I developed with the laboratory Thread & Lift, medical facelift by suspension threads brings permanent results worthy surgical facelift while maintaining a completely natural and smooth appearance to the face. The face is never expanded with the threads, unlike filler injections because they do not add volume.

Such results allow us to observe a rejuvenating of 5 to 10 years of facial appearance, through their action on all the sagged areas:

The facial thread lift will benefit these 4 areas and will have a very slight influence on the neck. However, it is possible to go further by specifically treating the neck by positioning 2 to 4 permanent suspension threads in this area.

It is now also possible to treat the upper third with suspension threads. They allow to perform an eyebrow lift. This operation can be performed independently or in addition to the facial thread lift.

The skin regains its tone, lost over the years, and is plumped through better vascularization of the face

Indeed, while lowering, hypodermis (superficial facial fat) causes the blood vessels to go down with it. These vessels communicate with the skin (epidermis and dermis) and feed it. But the collapse of the hypodermis stretches these vessels, which mechanically causes a reduction in their diameter. The ldrop of irrigation capacity costs the skin its quality and tone. By repositioning the hypodermis, tensor threads will then allow these vessels to return to their former diameter and irrigate properly again the skin to restore it to a much higher quality. (The effect just described usually occurs between 3 and 4 months after the thread lift.) Medical thread lift therefore has a very positive effect on the skin vascularization unlike the surgical facelift sectioning a large number of vessels.

Guaranteed results over time:

The repositioning of the sagging tissue is done by installing tensor threads on each side of the face (the cheeks). To ensure long-lasting results, it is necessary that those threads do not collapse as time goes.

To do this, we now have a permanent French-made tensor thread called Infinite-Thread®. This thread is made of ultra-resistant materials and includes more than 800 notches (4 every 1.5mm) made using patented technology. This very large number of notches, their conical “anti-rotation” shape and their orientation (8 traction axes instead of 4) offer the same power to hold sagging tissues as a surgical facelift. This ability to catch on has two major consequences:

  1. The first is the possibility of treating with the same quality of result light ptosis such as significant ptosis and heavy skin.
  2. The second consequence is the maintenance of the results in the very long term thanks to the hooking power of thread’s cogs, preventing the collapse of the tissues.

The duration of efficacy is proven to date of 10 years.

10 years

We are talking here only of the hindsight due to effictiveness. Regarding safety, the hindsight is well over 50 years. Indeed, the biocompatible components making the permanent thread are the references used in medicine and surgery from the 1960s, as we explain a little further detail.

I want to answer the following question, which is often asked: “What to do with the threads after those 10 years?

First of all, it is necessary to specify that this figure of 10 years is not a barrier and does not mean that the effect of the thread lift would stop sharply after the 10th anniversary of the intervention. This is an observation from my experience, concerning the average durability of the results enjoyed by the first patients who benefited from the combination of a thread and a quality installation technique in the mid-2000s. Of course, the quality of the suspension threads and techniques has improved even further since then.

Moreover, the threads do not disappear after 10 years. Indeed, the suspension thread is called “permanent” because it is made of biomaterials that the body does not eliminate in the short or medium term. This is the difference with so-called “absorbable”. It will not be eliminated by the body for several decades. It is not uncommon to find these intact biomaterials after 25 to 30 years in the body. The thread does not lengthen over time either. The quality of the result will therefore not be questioned by the alteration of the thread for decades.

However, no aesthetic intervention produces an eternal result. The same applies to thread lifting. So, where does this come from since the threads are not destroyed by the body? The reason is simple. As with a surgical facelift, the thread lift will treat the areas that need it. On the day of the procedure, the practitioner will raise the collapsed areas. The threads will be implanted in these areas. Therefore, areas that are not treated because they have not yet collapsed will be subject to this phenomenon in the future. They are the ones who will give the feeling of loss of result afterwards.

It will then be sufficient to complete the act that was done at the time, by adding 1 thread on the area or areas to be repositioned.

It must be understood that it will never be necessary to remove the old threads, no matter how many years have passed!

The Parallel technique, a non-surgical and safe procedure:

The procedure is non-surgical, it is carried out using needles with non cutting tips. This means that it requires no scalpel and therefore poses no risk of scarring. Similarly, the skin will absolutely not be devascularized with this medical facelift, in contrast to a skin undergoing a surgical facelift requiring a detachment with scalpel during surgery.

Moreover, the procedure is performed under local anesthesia in an operating room dedicated in the office, and therefore requires no hospitalization. It does not imply the significant risk of general anesthesia for the patient.

Finally, the perfectly bio-compatible thread ensures the impossibility of any allergies.

The facelift by permanent threads shows no indication against-sun exposure, even directly after surgery

The tensor threads have absolute resistance to the practice of all sports, even the most extreme (eg Martial Arts, Boxing, Diving, Winter Sports, etc.)

The tensor threads also have absolute resistance to all unpredictable shocks of everyday life (eg falls, hits the face, etc.)

Even not talking about breaking, all these external injuries will not be able to loosen the threads.

Moreover, tensor threads never impede the realization of a scanner or MRI. They do not affect, neither the practice of a medical examination (without exception) nor the reading of the results. In case of emergency surgery after an accident, the threads never pose any problems to the doctor or surgeon. These threads do not slow or hinder in any way the intervention in its effectiveness as in its quality of execution.

I now use the technique called “Parallel”. It is an even simpler technique than the Infinite-Lift technique, which provides even better results and comfort for the patient.

The Parallel technique no longer involves joining the threads at the temple, nor a knot at the top of the scalp. The adherence quality of the Infinite-Thread® over time makes this unnecessary. Each thread is independent with the Parallel technique. The procedure is therefore faster and less burdensome for the patient. This makes the procedure less invasive, less painful, limits the consequences and reduces the operating time to only 45 minutes for an even more effective result.

The parallel technique also allows a much better opening of the look. This gives us an incomparable good-complexion effect. This is an innovation compared to the already excellent results of the Infinite-Lift technique.

The Parallel technique offers record durability of results even without retensioning the threads, as it was already possible with the former Infinite-Lift technique. Once necessary every 3 or 4 years, this procedure is no longer useful to maintain the perfect effectiveness of the thread lift. Thanks to the power of the Infinite-Thread®, the quality of the results is maintained without further intervention, making the notion of retension completely obsolete.

This ability to maintain the quality of the initial result over the years is unique to date, whether in medicine or facial rejuvenation surgery.

In addition to the immediate rejuvenation of 5 to 10 years, the difference in perception of aging continue to grow each year in comparison with a person who has not benefited from the thread lift with permanent threads. Thus, if the appearance looks 10 years less at 60, it will be almost 15 years at 70. This phenomenon, known medically as “differential aging” is due to two reasons:

  1. The upcoming collapse of tissues will now be blocked or at least very greatly hampered by the presence of the threads. They will help the fibrillar system to support the subcutaneous tissue of the face, to fight against his own loss of quality due to years and gravity.
  2. The quality of the skin of the middle third of the face will be greatly improved thanks to a newfound vascularization, as explained in detail above.

This evolutionary gain, called differential aging, shows that the permanent thread lift is a preventative solution for patients under 40 years with mild ptosis who wants to limit the upcoming sagging of subcutaneous tissues.

The insurance of natural result:

Tensor threads are placed under the skin in a vertical axis. They therefore reposition the subcutaneous tissue in the upper and middle third of this same axis. Actually, this axis is the one that followed this tissue in the youth’s face. This restores a natural youthful appearance to the patient without anyone being able to notice the procedure. This advantage is made possible because the procedure does not require incision and never creates any scar. Indeed, unlike this medical thread lift, surgical traditional facelift requires pulling tissues and skin obliquely to hide the scar behind the ear, where it shows the least. Unfortunately, such an oblique axis sometimes brings unnatural result.

Similarly, surgical facelift is acting away from the cheekbone and the nasolabial fold by pulling the skin and subcutaneous tissue since the incision area made before the ear: it will therefore never have the effectiveness of the threads which are implanted directly into the collapsed area in the immediate vicinity of the groove.

No enlargement of the face :

This medical facelift by permanent tensor threads also ensures a natural result because it never expands facial features, which can unfortunately be found in case of significant injections of fillers.

Neither dimple nor wrinkles :

Finally, the threads never cause dimples in the cheeks, nor crow’s-feet wrinkles, whatever the type of tensor thread used, if the installation is performed correctly.

No alteration of facial features, at rest or in motion :

The natural facial features will never change in any way after receiving medical facelift by tensor threads. The threads will never create any distortion in the static or dynamic features of the patient’s face. This valuable benefit has two explanations. The first, ensuring the face at rest to maintain its natural, is the lack of incision during this facelift thus never altering any muscle structure, fat or nerve, unlike surgical facelift. The second explanation, ensuring the face in motion to retain its natural, lies in the layout plan located above and not in the facial muscles. These are never impacted by the presence of the threads and their actions are completely free. For example, if the patient’s face has a dimple it will of course still be present after the intervention. The thread simply cause a facial rejuvenation without changing its characteristics.

A discreet intervention:

The perfectly natural result of this technique allows the patient to not talk about his thread lift. This will only be known by him or her and her doctor. No need to explain or to justify to family or professional entourage.

This discretion is guaranteed thanks to:

  • Always invisible and painless threads under the skin, even to the touch;
  • The lack of “bump” under the skin making it impossible to grip your comb, the one of your hairdresser, or for your dentist to notice. Whoever manipulates the skin of the cheek, even for a long time and in a repetitive way, permanent threads will remain completely undetectable (obviously also with security gates, the threads being not made of metal);
  • The total absence of scar and bruise;
  • The speed of the procedure (3 hours of presence in the cabinet);
  • The lack of hospitalization;
  • The total disappearance of edema after 15 days (NB: Some patients experience persistent small swelling, in the worst case, up to 4 weeks after the procedure, without this being an handicap and especially of course without pain);
  • The rapid resumption of activity after only 3 to 8 days.

French training and thread :

  • Dr. Foumentèze having completed more than 1 000 thread lifts since 2002 and beeing lecturer at the Paris Faculty of Medicine.
  • The suspension thread I use is named Infinite-Thread®. It is born of thought and expertise of French engineers and doctors, and exclusively manufactured in France. The quality of the result depends as much on the thread, the laying technique, as on the competence of the practitioner. Without training, it will tend to move towards simplistic or inappropriate procedures.

100% neutral and gentle thread : Infinite-Thread®

A thread made of 100% neutral materials

The permanent suspension thread named Infinite-Thread® has an absolute chemical safety for the human body. Materials in the composition of this thread are exactly the same as those for surgical sutures used for over 50 years. The hindsight of more than half a century leaves no doubt about their safety. These materials are called “bio-compatible”.

The body always accepts and without exception the thread. It never “works” against it. Its tolerance towards the thread is complete.

A soft and gentle thread

Infinite-Thread® has effective notches, without ever being aggressive for the patient’s body. These notches measure less than 1/2 millimeter thick.

The temporary inflammation of the face following the intervention is the result of the set up procedure and not of implanted thread. The thread never causes any traumatic reaction from the body. That’s why, despite the presence of a thread under the skin of the face, inflammation is only temporary.

A “tailor-made” precision

The coating of the thread, made of solid silicone, ensures a very great flexibility. This allows to naturally accompany every movement of the patient’s face.

Moreover, the thread that I use is deliberately non-elastic. Therapeutically first, elasticity is of no interest since the facial tissues never extend whatever the mimic performed, even downwards. Studies have proved it but to convince yourself, you can measure the difference between any 2 marks on your face and you will see that it always keeps unchanged. In terms of accuracy, the non-elasticity improves the fine tuning of the thread tension, facilitates it and makes it much faster.

The laboratory has intelligently colored its thread in purple in order to ensure an easy finding in case of removal. It is indeed a color preventing any mistake with human tissue, unlike a transparent thread or white. Moreover, the thread is also perfectly visible on the ultrasound.

This guaranteed location as well as the quality of the materials make the removal of the thread  always possible easily, without any time limit, without sequelae and especially without leaving any residue under the skin.

A technique for all without the consindications of surgery:

  • Women and men,
  • People from 30 to more than 65,
  • Smoker and non-smoker,
  • Any type of skin: light or dark (Black, Mixed, Asian)
  • Every areas of the face : cheekbones (nasolabial groove), jowls (bitterness fold and oval disruption), neck, eyebrows, forehead,
  • People who have already benefited from a surgical facelift,
  • People who would like to benefit from a surgical facelift afterward,
  • Etc.

Permanent tensor threads are perfectly suited to sagging of tissues caused by the time …

The weight of years is an expression unfortunately entirely appropriate for facial tissues. Past 35/40 years, cheekbones starting to vanish, the grooves appear and the oval is lost under the weight of gravity dragging the fatty tissue under the skin down the face.

The only solution is to reposition these tissues to their original location, while pulling them up to the middle third of the face. This is precisely the action that permanent suspension threads will have on these tissues, without surgery.

… but also to subsidence affecting young tissues, much earlier than normal.

We said above that the absorbable threads were often adapted to this age group, whom ptosis of tissues is just emerging. Precisely, we are not talking here of people belonging to the majority of the population but the patients suffering since their youth from a morphology which does not allow them to enjoy an oval they like, or still giving them the impression of “puffy” cheeks despite a quite standard weight. For them, sagging tissue responsible for these shortcomings is not the result of time but of their original morphology.

In this case, the permanent thread lift makes a perfectly appropriate response. It will offer them correctly repositioned traits and keep this result permanent.

No cons-indication except deep radiofrequency and focused ultrasound (HIFU)

Thread lift has no cons-indication in terms of medical safety. The tensor threads do not have the usual cons-indications for surgery. The use of laser to treat brown spots, red, or rosacea knows no restriction.

The use of deep radiofrequency (active on the hypodermis) is contraindicated because it has the effect of heating the tissues and could cause the threads to lose their ability to catch (in principle, the latest generation of radiofrequency devices only act on the dermis and are therefore not contraindicated). Also, focused ultrasound is prohibited after thread lifting because it could burn the thread at its focusing points.. This remains a cons-indication in terms of efficiency and not in terms of safety, which is always 100% guaranteed.

The procedure: before, during, after

Before the thread lift

  • No aspirin or omega 3 (in the form of food supplements) during the 10 days preceding the intervention;
  • Antiseptic shampoo;
  • Plan to be accompanied for return.

During the thread lift

  • Patient’s presence in the cabinet for 3 hours (including 1 hour of intervention);
  • Intervention under local anesthesia and without surgery;
  • Intervention in the dedicated operating room of Dr. Foumenteze’s office.

After the thread lift

  • Be quiet for 24h;
  • Wait 24 hours to wash the head;
  • For facial cleansing: movement from the bottom up the first day after;
  • Potential embarrassment for chewing during the first 3 days;
  • Pain of the first 3 days felt only slightly because very easily treated by simply taking painkillers.

Pain management

Medical outcomes

Oedema is a simple swelling. It should not be mistaken for a bruise. The placement of the tensor threads in the subcutaneous tissue of the face implies edema around them but never any bruise or hematoma.

The two causes of this oedema:

  • The volume of the injection of local anesthesia (necessary anesthesia for not having pain or hematoma during the procedure); and
  • The inflammation created by the passage of the thread (regardless of the type of thread: permanent or absorbable).

The evolution of this swelling:

  • During the first hours, the oedema is due to anesthetic liquid and the inflammation. They make the subcutaneous tissue more spongy and the hooking of the threads remains fragile (the patient should not touch his face). This drawback is actually an advantage because it allows the physician to perform an ultra-precise adjustment of the tension of the thread, allowing an appropriate immediate loosening of the thread if it appears to be too tense. During the first day, the swelling affects the whole face balanced. Cheekbones are more prominent than the cheeks.
  • On the first evening, the anesthetic liquid will be gone and there will only be the simple oedema. The threads now hold perfectly and the risk that the patient untightens them suddenly no longer exists. But in the early days, if necessary, the doctor will still easily be able to fix an over-tension with a massage on very precise point.
  • During the 2nd and 3rd day, edema decreases. This decrease, irregular, will first appear in front of the passage of the threads. This is the largest decrease in that area which may give the impression to the patient, in some cases, they see the path of the threads at the lower end of his cheeks. In fact, he sees an area where the edema is absorbed faster, because of the weight of the threads on it. This situation is absolutely standard in the disappearance of the swelling process.
  • 4th day until the seventh day on average, edema will obviously continue to decline. However, it is important to be aware that it can vary with the days and at different times of the day. It is usually positioned higher in the morning and lowest in the evening due to gravity. Similarly, it is not uncommon for the swelling to be a little more visible in case of severe fatigue, or during the resumption of activity. These phenomena are completely normal in connection with the recovery and do not last.
  • After one week, 80% of edema has vanished and the threads are definitely attached. If it then proves that a thread is too tight, the doctor will fix it by re-injecting along the thread with an anesthetic liquid to recreate a hyper hydrated area around it to partially release it.
  • After 2 weeks, edema normally completely disappeared.
  • During weeks 3 and 4, it is possible that some patients experience persistent little edemas before they disappear completely. Micro defects (hollows) may also be visible during this period with a gradual improvement. These micro hollows will only be detectable by the patient himself, so he precisely knows his face and as it will, rightly, look at the slightest fault in the mirror. It is not uncommon that the patient believes discover a defect on his face at this stage that we find in fact on his photos from before the thread lift.

The most frequently expressed reluctance by your surgeon and their explanation:

In good faith, it would be difficult to prove him wrong since there were many disappointments since their appearance in France in 2002. The first threads promised then a nice lifting effect. Your surgeon has certainly attended the initial enthusiasm for the practice. He heard the speeches claiming provide answers to even young women, active, not wanting to receive surgical facelift.

Unfortunately for the truth of 2016, as many new features, the development was not ready in 2002 and the quality of results was not at the rendezvous before 2006. These four years of wandering have understandably finished to bury the reputation of thread lift. Critics of patients and surgeons rang out from all sides and the vast majority of practitioners willing to give up their scalpel to go forward did not show interest any longer.

This mature treatment, with a hindsight of more than 10 years, effective, sustainable and totally safe still suffers from the beginnings of the previous decade.

Again, your surgeon tells the truth. He definitely made poses that were unsuccessful, despite his great qualities and his habit of practicing much more complex interventions.

This undeniable truth has, however, rational explanations that do not call into question the quality of your plastic surgeon.

The prevailing vagueness around this technique, maintained by the clumsy communication of some laboratories, explains the inability of surgeons to distinguish a good from a bad thread. The latter being very present on the market. It so happens that, despite the best intentions, the chosen thread has insufficient quality to medium or long term and that disappointment comes quickly for the patient.

Beyond the highly questionable quality of some threads, training deficiencies are the main reasons for the opinion of your surgeon. The training provided by some players in the field does not allow your surgeon to be learn the best procedure. It is not a lack of will or competence on his part but a questionable politicy upstream. Indeed, during the training, with very (too) few exceptions, only the most simplistic techniques are presented. This allows for shorter numerous sessions, increasing the number of trained surgeons but reducing the quality.

This lack of training and the communication strategy making the procedure look trivial, especially with phrases like “lunch-time lift”, discredit the threads in the eyes of your surgeon .

This is all the sadder that it would suffice to demonstrate the objective superiority of the right procedure, taking the necessary time, and choose the best tensor thread to present to your surgeon so that it changes radically of opinion .

Third and final point often emphasized by surgeons, the visibility of the threads is not the result of an unjustified denigration.

It is actually noticed too often by surgeons on the face of their patient. This visibility is the logical consequence of a thread placed too superficially under the skin.

Certainly this visibility poses no risk to the patient and can be quickly corrected. The thread lift being fully reversible, face simply returns to its original state after the removal. Nevertheless, we must not overlook the importance of the disappointment caused to the patient, the value of time lost unnecessarily during withdrawal, and reputation consequence for the surgeon.

Again, this error of judgment in the selection of the thread depth positioning results from a lack of training. Insufficiently detailed, laying technique is not mastered by the novice installer. It will therefore have to experience later on. Quite humanly, he prefers then put the thread very surperficially by fear of going to deep into the muscle.

As long as this situation persists, some patients will continue to meet this concern. This is an additional argument for strengthening militant training but also against the oversimplification of the terminology surrounding this medical facelift.
